Application for Sick Kid -French Word Doc

The Lions Sick Children's Fund offers the following financial assistance:

Travel in order to receive necessary treatment
Travel costs including:
         Accommodations
         Tolls
         Parking
         Gas or Commercial Transport

Medical equipment
We can assist with medical equipment either in full or partial payment depending on the cost of the item and the Fund's financial situation.

Medications
We will assist with prescription drugs in certain cases:

Diabetic Test Strips:

Subject to availability of funds, we will assist families with the purchase of diabetic test strips for their child.

Disclaimers:
1. We will not duplicate payments if another agency is responsible for your medical expenses (Special Needs, Social Assistance, Insurance, etc).
2. Continuous assistance (Drugs, Supplies, etc) will be reviewed on a regular basis.
3. The FUND reserves the right to refuse initial requests for financial assistance, or to discontinue financial assistance.
4. We reserve the right to set a monthly maximum on continuous assistance.
